Assessment Tools

Below are assessment tools: history forms, rating scales and checklists. Each tool has a different purpose. Subscribers have access to their private myADHD Control Panel which enables you to send tools electronically. You will automatically be notified when the tool has been completed. The results will be encrypted, saved in your private user account and can be viewed or printed at your convenience when you log on to myADHD.com and go to your myADHD Control Panel. Completed assessment and tracking tools sent to your private account will be saved in your account through the term of your subscription unless you delete them. You can also print them or download them as a PDF file.

The Vanderbilt Assessment Scale—Parent Informant (6175)
This is a 55-item rating scale that has two components: symptom assessment and impairment of performance at home, in school, and in social settings. This rating scale is widely used by healthcare professionals to screen for symptoms of ADHD, oppositional defiant disorder, conduct disorder, and anxiety and depression in children.

The Vanderbilt Assessment Scale—Teacher Informant (6176)
This is a 43-item rating scale that has two components: symptom assessment and impairment of performance at in school. This rating scale is widely used by healthcare professionals to screen for symptoms of ADHD, oppositional defiant disorder, conduct disorder, and anxiety and depression in children.

SNAP-IV Teacher and Parent Rating Scale (18-items) 6160
This is an 18-item norm-referenced checklist that is designed to determine if symptoms of ADHD are present. This checklist can be completed by either a parent or other caregiver or an educator for use by a healthcare provider in performing an assessment.

ADHD Symptom Checklist (Child and Adolescent Version) 6177
This is a 21-item checklist that is designed to determine if symptoms of ADHD are present—in what settings such symptoms appear, when symptoms began and whether the child is impaired as a result of having such symptoms. This checklist can be completed by either a parent or other caregiver or an educator for use by a healthcare provider in performing an assessment.

ADHD Symptom Checklist (Adult Version—Self-Report) 6182
This is a 21-item checklist that is designed to determine if symptoms of ADHD are present—in what settings such symptoms appear, when symptoms began and whether the adult is impaired as a result of having such symptoms. This is the self-report version to be completed by the person suspected of having ADHD.

ADHD Symptom Checklist (Adult Version—Observer Report) 6183
This is a 21-item checklist that is designed to determine if symptoms of ADHD are present—in what settings such symptoms appear, when symptoms began and whether the adult is impaired as a result of having such symptoms. This is the observer report version to be completed by someone who know the adult suspected of having ADHD.

Adult ADHD Self-Report Scale (ASRS-v1.1) Symptom Checklist Form 6184
The ASRS v1.1 can be used as a tool to help screen for adult ADHD patients. Insights gained through this screening may suggest the need for a more in-depth clinician interview. The questions in the ASRS v1.1 are consistent with DSM-IV criteria and address the manifestations of ADHD symptoms in adults. Content of the questionnaire also reflects the importance that DSM-IV places on symptoms, impairments, and history for a correct diagnosis.

Child and Adolescent Developmental History Form 6180
This form asks about the child or adolescent's history and includes questions about family composition, current concerns, developmental and medical information, family information, school information, child management techniques used in the home and the child’s strengths and accomplishments. This form can be completed by a parent or other caregiver for use by a healthcare provider in performing an assessment.

Adult Psychosocial History Form 6189
This form should be completed by adults who are seeking an assessment. The form includes questions about current concerns, historical information from childhood/adolescence, health history and current status, educational, occupational, and social history, and there are checklists covering symptoms of ADHD, anxiety, and mood disorders. .
