Research Updates
ADHD in Preschoolers and Medication Response
A study reported in the November 2004 issue of the Journal of the American Academy of Child and Adolescent Psychiatry by E. J. Short and colleagues supports the benefits of stimulant medication for preschool age children. This study examined the safety and efficacy of varying doses of methylphenidate and mixed amphetamine salts (Adderall) with twenty-eight preschool children ages 4.0 to 5.9. Parent rating scale scores were used to determine the "best dose" of medication. Eighty-two percent of the ADHD children improved in their behavior by one standard deviation and over half improved by two standard deviations. Normalization of functioning was accomplished by 82%. The investigators reported an increase in side effects during baseline and placebo than at the best dose. Overall, the study findings demonstrate short-term efficacy and safety for stimulant treatment in preschool age children with ADHD.

Detection and Referral of Young Children with Developmental Delays.
Laura Sices, MD and her colleagues conducted a study to describe physician referral practices for children with developmental language delays and to test whether the probability of referral is increased by any of the following factors:
1. parent expression of concern about the child's delays
2. the gender of the child with a language delay
3. the behavior of the child (either avoidant or disruptive)
4. physician characteristics, including gender or specialty
A cross-sectional survey was sent to 800 pediatricians and 800 family practice physicians. Physicians answered questions about the likelihood of referral on a 5-point scale for listed referral options. Fifty-five percent of pediatricians and 43% of family practice physicians returned the survey. The results indicated that a girl with language delay was 60% more likely to be referred to audiology than a boy and respondents who were female or pediatricians were more likely to refer patients. The degree to which the family expressed concern about the delay did not increase the probability of referral. Children who were avoidant rather than disruptive in their behaviors were more likely to be referred.
