Research Updates
Study examines benefits of using atomoxetine (Strattera) to help children and adolescents with ADHD and comorbid oppositional defiant disorder.
ADHD is among the most common psychiatric disorders of children and adolescents and is often associated with difficulties in behavioral, social, and academic functioning. Up to 60% of children with ADHD also have impairments associated with comorbid mood, anxiety or behavioral disorders. The most common comorbid condition is oppositional defiant disorder (ODD) which is prevalent in 40% to 60% of children with ADHD.

Although no medication is currently approved for treatment of ODD, several studies have examined the efficacy of stimulants in the treatment of ODD, usually in the context of co-occurring ADHD. In the Multimodal Treatment Study of Children with ADHD, ODD symptomatology, as assessed by the SNAP rating scale showed improvement with stimulant treatment.

Less is known about the efficacy of nonstimulant medications in treating comorbid ADHD and ODD. Atomoxetine (Strattera) is a nonstimulant agent approved by the U.S. Food and Drug Administration for the treatment of ADHD in children, adolescents, and adults. A recent study by Newcorn et al. (2005) examined the effects of atomoxetine on ODD symptoms in a sample of children and adolescents ages 8 to 18 with ADHD and ODD. They found that youths with ADHD and comorbid ODD showed statistically significant improvement in ADHD, ODD, and quality-of-life measures. Treatment response was similar in youths with and without ODD, except that the co-morbid group showed improvement at 1.2 mg/kg/day and no incremental benefit at 1.8 mg/kg/day. The study investigators concluded that atomoxetine treatment improves ADHD and ODD symptoms in youths with ADHD and ODD, although the comorbid group may require higher doses.
