Instructions
Each of the tracking scales below has a different purpose. Read the description of each scale then select the one you would like to use. You can view the scale and print it, download it as a PDF file, or send the scale electronically from your Subscriber Administration Page. This is our most popular option and it is unique to myADHD.com. From your personal Subscriber Administration Page you can view, complete, send and receive scales online using your private, secure subscription account. You will automatically be notified when the scale has been completed and returned to you. The results will be encrypted, saved in your account and can be viewed or printed at your convenience when you log on to myADHD.com and go to your  Subscriber Administration Page. Completed scales sent to your private account will be saved in your account through the term of your subscription unless you delete them.

ADHD Symptom Checklist (Child and Adolescent Version) 6177

This is an 18-item checklist that is designed to determine the degree to which symptoms of ADHD are present. This checklist can be completed by either a parent or other caregiver or an educator for use by a healthcare provider in tracking the child’s progress.

This is a 21-item checklist that is designed to determine if symptoms of ADHD are present—in what settings such symptoms appear, when symptoms began, and whether the adult is impaired as a result of having such symptoms. This is the self-report form to be completed by the adult suspected of having ADHD.

ADHD Monitoring System (Instructions and Weekly Monitoring Form) 6179

The ADHD Monitoring System was designed by David Rabiner, Ph.D. to carefully track a student’s progress in school so that parents and health care professionals will be alerted when any adjustments or modifications to treatment need to be made. This packet contains a monitoring form and the instructions needed to interpret the information this form provides.
